I give Laury Rappaport, Ph.D., LMFT, REAT, ATR-BC, and the Focusing and Expressive Arts Institute the right and permission to copyright and/or publish, alter, reproduce, or otherwise use my artwork, voice, and likeness in video, photographs, written materials, and audio-visual recordings. I acknowledge and understand these materials about or of me may be used for educational and promotional purposes– such as FOAT® or FOAT®-related courses and trainings, professional presentations, newsletters and email promotional material, or on any Focusing and Expressive Arts Institute platforms (including website, social media, blogs).

I understand that my participation is voluntary and that if I wish to discontinue my involvement so that my likeness is not recorded visually or audibly, I take sole responsibility for turning off my camera and muting my microphone when taking part in any future online teleconferences or meetings with any associated parties. In addition, the only way that I may revoke my consent to this release form is if I provide written notice to all involved parties within (3) days of initial submission. If I do not revoke consent within the allotted time, I accept there is no time limit on the validity of this release nor are there any geographic limitations on where these materials may be distributed.
